- Upper path: Go through the Middle Hollow Shrine, then jump down to a ledge guarded by Skeletal Snakes.
- Lower path: Go to the Bonevault Shrine, this path will lead straight to Centipede Guai’s lair.
For a “smooth” battle with Centipede Guai, careful preparation is extremely important. Here’s what you need to keep in mind:
- Poison resistance: Make sure you bring enough Curios or Antimiasma medicine. Centipede Guai has the ability to spit extremely annoying poison, and being poisoned will cause you to lose health continuously.
- Stance: Choosing Pillar Stance is not a bad idea. This stance helps you easily dodge the poison pools on the ground created by Centipede Guai.

Head Slam: Centipede Guai will slam its head to the ground twice. Dodge to either side
Stand Up and Spit Poison: When Centipede Guai stands up, move back quickly. It will spit poison and create a large poison pool. Use Pillar Stance to walk on those poison pool. Wait the pool to disappear, then you can attack.
Curl Up and Rush: This is the most dangerous attack. Centipede Guai will curl up and rush straight at you at high speed, possibly hitting the wall and attacking again. Time it precisely and use Rock Solid to make it fall. This is a good opportunity for you to unleash powerful attacks.
Curl Up, Fly Up, and Slam Down: When Centipede Guai’s health is below 50%, it will have this additional move. It will curl up, fly into the air, and slam down. Be careful when trying to parry, because immediately after slamming down, it will swing its claws to attack.
Summon small centipedes: Also at health below 50%, Centipede Guai will roar and summon a swarm of small centipedes. They will roll towards you and explode on impact. Run in the opposite direction of the Centipedes swarm, then dodge to the side so they collide with each other and explode. Be careful: After summoning the centipede swarm, Centipede Guai will rush at you at a faster speed.
After defeating Centipede Guai, you will receive the Centipede Guai Spirit – a transformation spell that allows you to transform into Centipede Guai.
